OHV Enforcement and Dust Control Reporting Forms

The Arizona Department of Transportation (ADOT), cities, towns, counties, and off-highway vehicle (OHV) enforcement departments/agencies are required to submit a report by March 30 each year regarding activities related to particulate matter air pollution controls and strategies implemented in Area A1 to ADEQ for the previous calendar year.2

Reporting Forms

Please only fill out and submit the form applicable to your organization. Each entity should submit only one form.

How to Submit Reporting Form

The reporting form is due by March 30 each year and covers activities for the previous calendar year (e.g., forms submitted by March 30, 2026, will cover calendar year 2025 activities).
