[Ariz MSGP 2010] New MSGP 2010--related forms

The following MSGP-related forms are now available at
http://www.azdeq.gov/environ/water/permits/msgp.html  :

 

*	Annual Report Form 
*	Discharge Monitoring Report Form 
*	Sector L Certification Form - this form is to be used ONLY by
facilities that never had coverage under the MSGP, but must file for
closure under the AZPDES stormwater program. 

 

These forms are set up to either fill out on-line and print or print a
blank form and complete manually. Which ever method you chose, the
completed forms must be mailed or faxed with an original signature to
the address in Part 7.6 of either permit. Please disregard the note on
the Annual Report Form stating that "Faxed forms are not acceptable".
