Aquifer Protection Permit (APP)
Arizona Pollutant Discharge Elimination System (AZPDES)
Individual Permits
- Basic Information (Form 1) - Nature of business, name, mailing address, location, existing permit numbers, etc. Used in combination with Forms 2B, 2C, 2D and 2F.
- Publicly Owned Treatment Works (POTW) and Domestic Wastewater Treatment Works (Form 2A) - Publicly-owned or any other treatment works that treats domestic sewage and currently discharges, is permitted to discharge, or proposes to discharge treated wastewater to waters of the United States.
- Renewal Application for Industrial and Commercial Operations permitted to discharge Non-domestic Wastewater (Form 2C) - Private or government owners of facilities that currently discharge or are permitted to discharge wastewater other than domestic wastewater to waters of the United States. Includes discharges from water treatment plants, groundwater remediation efforts, mining and silvicultural operations, and noncontact cooling waters among others. The form requires detailed information on location of outfalls, sources of intake water, production levels and detailed testing data for pollutants contained in effluent. Form 2C must be accompanied by Form 1.
- Initial Application for AZPDES Permits for Industrial and Commercial Operations that will discharge Non-domestic Wastewater (Form 2D) - Private or government owners of facilities that propose to discharge wastewater other than domestic wastewater to waters of the United States. Includes discharges from water treatment plants, groundwater remediation efforts, for manufacturing, mining and silvicultural operations, and noncontact cooling waters among others. Form 2D must be accompanied by Form 1.
- Dischargers of Stormwater Associated with Industrial Activity (Form 2F) - Facilities whose discharges are composed of stormwater and non-stormwater must submit Form 2F for their stormwater. Form 2F must be accompanied by Form 1.
- Facilities that treat Sewage Sludge (Biosolids) (Form 2S) - All Publicly Owned Treatment Works (POTW) and other facilities that treat domestic wastewater and facilities that do not treat domestic wastewater but treat or dispose of sewage sludge.
