View public records and files and make a records request by visiting the Records Center Monday through Friday between 8:30 a.m. and 4:30 p.m. You may also make a records request by mail, fax or e-mail.

When making a Records Request, please fill out the form. The form can be submitted by e-mail as an attachment, faxed or mailed.

  • (1) Your Name (Required)
  • (2) Your Phone Number (Required)
  • (3) Fax Number
  • (4) The Name of the Company or Group you are representing
  • (5) Your mailing Address
  • (6) Your Email Address
  • (7) Indicate how you would like to Review the Records (Digitally, printed, or in-person)
  • (8) Indicate if the Records will be used for a commercial purpose. If for a commercial purpose, indicate the value of the reproduction on the commercial market. (Required)

Help our Records Center staff locate requested files faster by providing any details you may know concerning the file, for example:

  • Current and/or Previous Site Name(s)
  • Address
  • ID Number (eg. RIMS, Site Code, Inspection Number, Licensing Time Frame (LTF) Number, Case ID, AZPDES, Inventory Number, Communication Tracking System (CTS) Number, Public Water System Number, Task Assignment Number, etc.)
  • ADEQ Program Responsible
