 SUPERFUND/WQARF PROGRAMS: FILE REVIEW PROCEDURES

The complete official site files are located in Phoenix at the ADEQ Main Office at 1110 W. Washington St. However, select documents are also available in Tucson at the Southern Regional Office at 400 W. Congress, Suite 433.

The Superfund Programs Section files are available for public review subject to the following guidelines:

  1. An appointment to review files should be scheduled at least one week in advance
  2. To schedule an appointment to review Preliminary Assessment or Site Investigation files, contact the Site Assessment Unit at (602) 771-4227 or toll free at (800) 234-5677
  3. To review WQARF, NPL or DoD files, contact the ADEQ Records Center at (602) 771-4380 or toll free at (800) 234-5677
  4. In order to identify the portions of the file you need to review, please request a file index at the time you schedule your appointment
  5. Public review hours are from 8:30 a.m. to 4:30 p.m.
  6. No briefcases, backpacks, folders, portfolios, etc., are permitted in public review areas
  7. If copies of the file or parts within a file are requested by an individual reviewing files, an ADEQ staff member will coordinate with our contracted copier service to arrange for all duplication copies required. Copies are $0.25 per page
  8. If a client/customer wishes to duplicate parts of or entire files using electronic image reproduction/scanner equipment, the client will be provided minimum space in the review area provided the client has submitted a written request at least one week in advance. The advanced written request is required for formal approval by ADEQ granting permission to the client to bring photographic/reproductive equipment into ADEQ's facility

  1. An appointment must be made to review the files. To schedule an appointment to review WQARF files, please contact the ADEQ Southern Regional Office at (520) 628-6715 or, toll free in Arizona, (888) 271-9302.
  2. In order to identify the portions of the file you need to review, please request a file index at the time you schedule your appointment. A staff member will fax or mail you a copy of the file index if time allows, and you will be asked to mark which files you need and send the index back before your appointment
  3. Public review hours are from 8:30 a.m. - 4:30 p.m. To review files, please check in at the ADEQ Southern Regional Office at 400 W. Congress, Suite 433, in Tucson, and you will be directed to the file review location
  4. No briefcases, backpacks or other bags are allowed at the public review tables
  5. If you wish to have copies made, ADEQ staff may copy up to 10 pages at no charge; if you would like between 10 and 24 copies, there is a charge of $0.25 per page. If you need more than 24 pages, you need to make arrangements with a bonded copy service. Suggested bonded copy services are Alphagraphics, Kinkos and Reproductions. The cost of the copies is dependent on the service selected
